Urgent Care Telehealth — $49 consultation Bloomfield Horizon LLC COMMON CONCERNS • Cold and flu symptoms, cough, fever, allergies and sinus concerns • Urinary tract infection (UTI) symptoms and other minor infection concerns • Heartburn, nausea and mild digestive symptoms • Rashes, insect bites and minor skin concerns • Medication refill requests WHAT TO EXPECT 1. Discuss your symptoms and medical history during a virtual consultation. 2. Receive a care plan based on your evaluation. Medication may be prescribed when appropriate. 3. Get guidance on follow-up, testing or an in-person visit if needed. The consultation fee is $49. Treatment and refills depend on clinical evaluation and are not guaranteed. Some concerns cannot be assessed or treated fully through telehealth. Mental health services are not offered. For a medical emergency, call 911.
